Rates, Policies, and Forms

 

In Office Counselling Rates :

 

          $125 per 50 minute individual session

 

          $190 per 60 minute couple session.  

          *Fees and Payment will be collected at the time of service.  All fees include GST where applicable.

Online Counselling Rates :

          $125.00 per 50 minute individual session.

      

         *190.00 per 60 minute session for Couples Counselling.

         *Fees and Payment will be collected at the time of service.  All fees include GST where applicable.

 

Insurance - Please consult with your human resources department or your insurance company to determine whether your employee extended benefit plan covers therapy provided by Registered Therapeutic Counsellors (RTC). 

 

Payment - Payment will be made, at time of service, by either cash, email transfer or cheque.  Receipts are provided and may be eligible for submission on your income tax.

 

Cancellations - If you find it necessary to cancel an appointment, please contact me at least 24 hours in advance. Cancellations with less than 24 hours advance notice will be charged the full price for the session.  I may also terminate counselling in the event the client has missed 3 appointments without calling to cancel 24 hours prior to the scheduled appointment.

 

Forms - Before the first session, please fill out an Informed Consent Form and Confidential Intake Form, available at the links below. 

 

You may print out the forms and bring them with you to the first session, or if you prefer, you may scan and e-mail the completed forms to me directly.  If neither of these requests is possible, I will provide the forms at your first session.
